Athlete and guest registration is a 2-step process:

  1. Complete Registration Form

  2. Sign your Waivers (only needed once a year)

***DISCLAIMER: If you have had hardware placement/instrumentation in the past 12 months, or are less than one year post injury, you are ineligible to participate unless you show written consent from your physician.***

Come back to this page after completing registration to sign your waivers

Step 2

We need everyone who is coming to the event to sign a Participation Waiver & Media Release; this includes Friends, Family Members & Caregivers. This only needs to be done once a year.
